Sublinguaal toegediende huisstofmijtextract 12 SQ-HDM Voorwaarde: uitsluitend voor een verzekerde met aanhoudende door huisstofmijt geinduceerde allergische rhinitis of rhinoconjunctivitis, gediagnosticeerd op basis van de klinische voorgeschiedenis en een positieve huisstofmijtsensitisatietest (huidpriktest en/of specifieke IgE), ondanks adequate medicamenteuze behandeling.
